Cisco 7960 VOIP
Additional Line Request
On the 7960 series VOIP Telephones, you may have up to 6 active lines. That gives you an additional 5 lines that you may use or monitor.
Please fill out the form below to request the additional line usage. This form must be attached to your TS1 form, if you are getting new service. If you are changing existing service, please fax this form to Telephone Services at 6120. The lines correspond with the numbered spots on the picture. If you need extra explanation on column, click on the column header that is underlined .
This form works best when completed in Internet Explorer, all others may not print correctly
